Invalid Rm listing for LDR, STR, and TESTSET

Specifically:

* LDR/STR (INDEX) (32)
* TESTSET (32)
* LDR/STR (PM) (32)

These have Rm0-2 listed twice in their decode rows. Presumably the listing from 13-15 are meant to say Rd0-2.
